The initial term of this Agreement is [6/12] months, beginning on the date above.

The Agreement will automatically renew on a month-to-month basis unless terminated in writing by either party with a 30-day notice.

Includes:
- This option allows clients the flexibility to choose any combination of treatments they desire, providing them with a truly personalized skincare experience.
- 15% discount on all skincare products
- 25% discount for any additional treatment

Note: This membership option excludes treatments containing exosomes and stem cell products.

3. Payment Terms

Membership fees are due monthly on the same date the Agreement was signed.

Fees will be automatically charged to the Member’s provided payment method each month.

Any additional services or products not included in the membership will be charged at the time of service.

4. Cancellation Policy

Members may cancel their membership with a 30-day written notice after the initial term.

Early termination of the Agreement within the initial term will incur a fee equal to [1/2] month's membership fee.

Studio reserves the right to terminate the membership at any time with a 30-day notice.

5. Booking and Appointment Policy

Members must schedule appointments in advance to ensure availability.

Cancellations or rescheduling of appointments must be made at least 24 hours in advance to avoid a cancellation fee.

6. Membership Benefits

Membership benefits are non-transferable and non-refundable.

Unused services do not roll over to the following month.

7. Health and SafetY

Members are required to complete a health and safety questionnaire prior to their first treatment.

Studio reserves the right to refuse service to any member based on health and safety concerns.

8. Liability

Studio is not responsible for any adverse reactions or outcomes resulting from treatments performed in accordance with the Member’s instructions or disclosed medical history.

9. Governing Law

This Agreement is governed by the laws of the State of Florida. Any disputes arising from this Agreement will be resolved in the courts of Florida.
